Billing Policy
Northwest Pediatric Associates has a contractual obligation with the insurance carriers, therefore, payment of co-pays, co-insurance, outstanding balances due and/or deductibles are collected at check in on the date of service. For private pay patients, the total amount due is collected at the end of the visit. Northwest Pediatric Associates files insurance claims on the date of service, and it is important that we have the most up to date insurance information at every visit, so please be prepared to present a verifiable insurance card at every encounter.
Fees for Non-Contract Services
Northwest Pediatric Associates does not charge for sport physicals or camp forms, but the child needs to be current with a yearly physical and required immunizations. Northwest Pediatric Associates does not charge for the first copy of immunization records, but there is a $10.00 charge for more than one copy.
There is a charge of $25 for FMLA forms, this fee must be paid every time that the form is filled out by the physician. There is also a $25 charge for a copy of your child’s complete medical record.
Ear piercing is a purchase that is not covered by insurance companies and must be paid at the time of service. Our fee is $25 and this includes the earrings.
There is a $5 fee for controlled substance prescriptions such as Concerta, Adderall, Ritalin, etc.
There is a $25 charge for any returned check. These returned checks and the additional charge must be paid in cash, with a money order or credit/debit card.
